We at Fairview Health Services are looking for a dedicated Licensed MI/CD Psychotherapist for our Lodging Plus Program located in Minneapolis, MN. This dually licensed psychotherapist position reviews assessments at time of admission, resolves clinical interventions, facilitates therapy and provides education for patients and their families. Facilitates treatment planning and leads each case. Responsible for development of transition/discharge plan. Communicate optimally and respectfully with others, including interdisciplinary teams and all other contacts. Functions as an interdisciplinary team member.

Schedule : This position is 0.80 FTE (64 hours per 2 weeks), 8 hour long day shifts with 1 weekend a month rotation.

Responsibilities Job Description

Demonstrates clinical competence in the treatment planning process.

  • Facilitates treatment planning and coordinates management of each case.

  • Coordinates/facilitates the development of treatment plan by collaborating with patient, family and interdisciplinary team.

  • Develops an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.

  • Implements, reassesses and adjusts treatment plan as necessary.

  • Completes an interdisciplinary assessment tool with each patient.

  • Updates interdisciplinary assessments' tool each week.

  • Assists in developing an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.

  • Implements, reassesses and adjusts treatment plan as necessary.

  • Communicates frequently and optimally with interdisciplinary team to ensure continuity and coordination of care.

  • Documentation is detailed, timely, legible and clinically appropriate and meets regulatory requirements.

  • Collaborates with patient, family, and provides input to interdisciplinary team in developing a discharge plan based on age, social and cultural considerations, clinical diagnosis and level of functioning and other relevant needs.

  • Demonstrates clinical competence in conducting individual therapy, group therapy, family therapy and psycho-education groups.

  • Provides individual, group, couples and family therapy.

  • Provides psycho-education groups

  • Ensures detailed, timely, legible and clinically appropriate documentation.

Demonstrates the ability to advance evidence-based practice by utilizing standards, guidelines and pathways for care delivery. Incorporates data and information into practice to continually improve care and practice so that patient outcomes are improved.

Qualifications

  • One of the following current Minnesota licenses: Licensed Independent Clinical Social Worker (LICSW), Licensed Psychologist (LP), Licensed Professional Clinical Counselor (LPCC), or Licensed Marriage and Family Therapist (LMFT)

  • Licensed Alcohol and Drug Counselor (LADC).

  • Master’s degree in psychology, social work, nursing or closely related field.

  • Basic Life Support certificate

Preferred

Experience

  • 2 years of supervised therapy experience in mental health/substance use disorders including assessment, individual, group and family therapy.
